The key roles of the CHW includes coordination of culturally competent patient care and services providing outreach to African American community members (Support and inititate community buidling activities). It will be imperative to support, encourage and empower patients via health education, one on one workshops and classes, referral and resource networking and working closely with WMC staff to advocate for the patients individual needs. The CHW is able to engage patietns to provide them with education and tools to make healthy lifestyle choices and manage their own health diagnoses.
Provide high quality, culturally appropriate health care for an at-risk, Medicaid, under-or uninsured patient population served by Wallace Medical Concern. The role involves patient assessment and implementation of a treatment plan for both acute and chronic conditions, in addition to well child care and women's health. This Physician will be part of a small, multi-disciplinary team. Care will include performing work-ups on patients to include a history and physical, collaborating with the patient and family/caregiver in developing a care plan. This position will document in EPIC all care and teaching provided in accordance with accepted medical/legal standards. Provide all medical care in accordance with accepted practice standards, following clinic protocols, and State of Oregon licensing guidelines. This position may include on-call responsibilities.
